Someone considered to be mentally frail (חולה תשוש נפש) is an individual suffering from cognitive decline including memory problems; and/or problems related to self-orientation with regard to time/location; and/or apathy/behavioral disorders that are liable to endanger him/herself. The emphasis in this definition is on the cognitive state of the individual, as someone who is mentally frail may be completely independent functionally without any special physical problems.

Example: 
A 78 year old man who does not know the day or time, does not remember the names of his family members and despite the fact that he is capable of eating on his own, does not initiate preparing food on his own.
